Who handles birth certificates

Births are registered with the Local Civil Registry Office (LCRO) of the city or municipality where the child was born: Malaybalay City, Valencia City, or one of the 20 municipalities of Bukidnon. The Provincial Government does not register births or issue birth certificates. Requirements and fees vary by city and municipality, so confirm with the office before you go. Two Ways to Get a Copy

Choose the office copy or the PSA copy, depending on what the requesting agency needs

From the City or Municipal Civil Registrar

  1. Go to the civil registrar of the city or municipality where the birth was registered.
  2. Fill out the request form: full name, date and place of birth, and parents' names.
  3. Present a valid ID. A representative also brings an authorization letter and the owner's ID.
  4. Pay the fee set by the LGU and claim the certified copy.

From the PSA

  1. Order online through PSA Serbilis for delivery, or visit a PSA Civil Registry System outlet.
  2. Give the owner's full name, date and place of birth, and parents' names.
  3. Present a valid ID if you go in person, and pay the PSA fee.
  4. Receive the copy printed on PSA security paper.

Registering a Birth

General steps. Your civil registrar may ask for more.

1

Get the Certificate of Live Birth

The hospital, clinic, midwife or birth attendant usually prepares the Certificate of Live Birth.

2

Check the Details

Check the spelling of names, the date and place of birth, and the parents' details before signing. Errors are harder to fix later.

3

Submit Within 30 Days

File it with the civil registrar of the city or municipality where the child was born, within 30 days of the birth.

4

Add Supporting Documents

If the parents are not married, the father's acknowledgment and an Affidavit to Use the Surname of the Father may be needed. Ask the civil registrar.

5

Pay Any Fees

Pay any fee the city or municipality charges at its treasurer's office.

6

Get Your Copies

The civil registrar can issue a local copy once the birth is registered. The record is then sent to the PSA, so a PSA copy is available only after some time.

Requirements

Commonly asked for. Requirements vary by city and municipality.

For a Certified Copy

  • Valid government-issued ID of the requester
  • Full name, date and place of birth, and parents' names on the record
  • For a representative: authorization letter and the owner's valid ID

For Delayed Registration (After 30 Days)

  • PSA negative certification (proof that PSA has no record of the birth)
  • Affidavit explaining the reason for the delay
  • Supporting documents showing the name and birth details, such as a baptismal certificate or school records

Frequently Asked Questions

Does the Provincial Capitol issue birth certificates?
No. Birth records are kept by the civil registrar of the city or municipality where the birth was registered, and by the PSA. The Provincial Government of Bukidnon does not issue birth certificates.
I was born in Bukidnon but live somewhere else. How do I get a copy?
Order a PSA copy online through PSA Serbilis or at any PSA outlet. For a local copy, contact the civil registrar of the city or municipality where you were born.
What's the difference between a local copy and a PSA copy?
Both are certified. PSA copies on security paper are usually required for passports, visas and many national agencies. Local copies are often accepted for local transactions. Ask the agency which one it needs.
PSA says it has no record of my birth. What now?
Ask the civil registrar where the birth was registered to check its records. If the record is there, the office can endorse it to the PSA. If the birth was never registered, you will need to file a delayed registration.
How do I correct an error on my birth certificate?
Clerical errors, a change of first name, and errors in the day or month of birth or in sex can be corrected by petition at the civil registrar under Republic Act 9048 and Republic Act 10172. Other changes need a court order.
